Building an Effective Incident Report SystemIt all starts with what sort of incidents are you looking to capture and report. What do you want for a follow on process once an incident has been submitted? How will incidents be signed off and closed? Will there be corrective actions? What sort of notifications do you want to setup?Step 1: What forms do you want in placeAs a starting point, what are the forms you want to establish. An incident report to capture an injury or illness? how to capture an accident in the workplace? Ask yourself if the incident will be broken up into smaller categories such as property damage, vehicle incident, injury or illness or even a security incident.For additinal forms, will you need them to have seperate registries? Environmental incident reports, hazard reports or other custom form types and registries. Step 2: Create your forms
Build the forms so that they are simple and easy to use. Classifyform questions by category such as the details of the incident, severity, category, type, investigation, actions and sign off.
Step 3: Build your registries
Now that you have your form workflow, each one you have a registry of submitted incidents to action. Determine how you want to categorise these registries by field type. This makes it easier to manage incidents and follow them up.
Step 4: Setup Incident notifications
Finally, define what type of notifications you want to setup for when an incident is submitted.
